Athletic Registration Forms - OSAA, Payment Plan, Insurance, Travel Release, NCAA

Fall Sports begin August 18th.

All Student Athletes need to be registered prior to practice or tryouts. We are asking the parents/guardians of all students planning to play sports to create accounts in FinalForms and register your student(s).

Please register at: https://forestgrove-or.finalforms.com/ and follow the prompts to create your account, create your students, and sign your forms.

Students are REQUIRED to be covered by health insurance to participate in Athletics.

School Insurance is available/required for purchase in the event of no coverage, please sign up at Myers & Stevens.com
